Please download our toolkit if you would like to support Queen Elizabeth Hospital Birmingham Charity. On the back page is a Fundraising Registration Form which we would ask you to complete and return to us before your event.

Grant Application Forms

Colleagues working at the Queen Elizabeth and Selly Oak Hospitals can download our grant application forms here to request expenditure from research funds, general funds or earmarked funds.
Expenses Forms

If you need to claim for reimbursement of expenses incurred on behalf of the charity, the forms and guidelines are available here Please note - this is for colleagues working at the Queen Elizabeth & Selly Oak Hospitals only, not for fundraisers. Fundraisers should contact our team on charities@uhb.nhs.uk if you have a query around expenses.
Fund Advisor guidelines

Fund Advisors can recommend expenditure on earmarked funds subject to the charity's guidelines. The latest version, dated June 2009, contains details on how to utilise the charitable funds.
